I hereby agree to release and discharge the Lakeland Art Association, it’s officers, employees, agents and subsidiaries from any and all liability for personal injury, property loss or damages by any person of any nature or cause suffered as a result of my participation in this event, have read and agree to be bound by the rules of the Artisan Holiday Show and, if accepted, give permission to use my name, business name and images of my work for advertising and publicity purposes.

Artisan Holiday Show
RULES AND REGULATIONS
Products for sale must be handmade by the artist.
Acceptance is at the sole discretion of the Lakeland Art Association (LAA) jury committee.
Allowed:Limited edition prints , T-shirts if hand dyed, hand printed, hand painted, etc.
Not Allowed: Products purchased for resale, Mass Produced Products, Party Plan merchandise, Commercially made products that are merely embellished.
If any items are determined to be unsuitable for the show they will be required to be removed. The LAA will have the sole determination of what is deemed to be unsuitable.
Artist is responsible for collecting and remitting sales tax when applicable.
Tables will be provided. Artist must provide own table coverings (to floor) and displays.
Mini white Christmas lights, though not required, are encouraged at all booths to give the show a cohesive look.
Artist is responsible for cleaning up their own area after show.
Artist is encouraged to have insurance as the Lakeland Art Association will not be responsible for damages or injuries as a result of artist’s participation in the show.
Checks for booth rental will not be cashed until artist is accepted into show
NSF checks to Lakeland Art Assoc. will incur a $35 service charge
No sharing of booths by multiple artists unless they actually produce the products as a team or are a 501c3 cooperative.
No jewelry allowed except by artists who are juried in on jewelry
Only Blue Painters tape allowed on walls, no nails permitted.
Artists requiring electricity will be required to provide their own extension cord(s). Any exposed cords must be taped down.
Cancellation Policy: Booth fees returned if cancellation is made by 10/19/18. 50% of booth fees returned if cancellation is made after 10/19/18.
